If you are a new patient, please bring with a list of all of your medications, including over-the-counter vitamins, minerals and herbal supplements that you regularly use.
If you are not fluent in English, please bring someone with to help translate, or inform us when you receive your appointment information and we will be happy to book an interpreter for you.
Unless told by our office, you will not be having a procedure done during this appointment, so there is no need to fast.
If you are coming for a procedure on your first visit:
Please print, read and sign the consent form PRIOR to arriving on the day of your procedure.
Dr. Bookman will talk with you, answer your questions, and examine you prior to your procedure.
Please note that you MUST have an adult (Uber or Taxi does not count) to accompany you home after your procedure, or else you will not be allowed to receive sedation. These are policies of the facility.
If you would like to meet with Dr. Bookman on a separate day prior to your procedure, please contact our office.

Please note there are different types of Preparation Forms.
If you go to the washroom fairly easily, do not take narcotics (pain killers), and do not have diabetes, please read and follow the directions on the Colonoscopy Preparation Form.
If you tend toward constipation or take narcotics (pain killers) or have diabetes, please read and follow the directions on the Colonoscopy Preparation Form.
If you have at least 2 of (1) Constipation, (2) Take narcotics (pain killers), (3) Diabetes, please read and follow the directions on the Colonoscopy Preparation Form.
Please follow the colonoscopy instructions above that are most appropriate for you.

If you are coming to the Kensington Clinic, you must complete the Preoperative Questionnaire.
Please fax or mail to us no later than 5 days prior to your procedure. Otherwise, the anaesthetist may cancel your procedure on the day of the test if they feel your health conditions make it unsafe to proceed outside of a hospital setting.
You must have an adult to accompany you home after the test. A taxi or TTC does NOT count. Your procedure will be canceled if you arrive alone.
